Problem:
After successfully adding and logging into an Exchange mail account with
Outlook 2003 starting Outlook may result in a window that says

      "Preparing Outlook for first use. Retrieving mailbox settings..."

This dialog never completes, leaving only a Cancel button, but it is greyed out.

Solution:
The solution to this problem is to uncheck 'Use Cached Exchange Mode' in
the account settings.
